FBA/BIP Turnkey Materials
Functional Behavioral Assessments (FBAs) and Behavior Intervention Plans (BIPs) are part of a problem solving process for addressing student behaviors. They can be used to identify the purposes of a problem behavior and identify interventions to directly address the problem behaviors. The materials were distributed at the turnkey training on April 8th, 2013 and can be used to provide PD to school staff around creating FBAs and BIPs as part of the IEP process.
Updated FORMS! - Completing FBA/BIP and uploading to sesis
Beginning January 15, school-based and regional committees on special
education (CSEs), and committees on preschool special education (CSPEs) should
no longer use the Functional Behavior Assessment (FBA) and Behavioral
Intervention Plan (BIP) forms that are currently in SESIS. Please use the forms below, which are also available on the DOE Intranet. The forms still need to be faxed into SESIS. In addition, the Considerations for Positive Behavioral Supports form must be completed at every CSE/CSPE meeting and faxed into SESIS.
